Please print and mail completed form, signed release from liability form and deposit to Midwest Monarchs, 686 Bethany Ln, Wentzville, MO 63385. Once we receive your information and deposit we will contact you and lock in your dates.  After the MO DNR publishes the dates we will again contact you and make any adjustments that may be necessary.  Please keep in mind we ask all clients to arrive by 12 noon the day before their start date to check weapons and receive an orientation from one of our representatives.  Remember, clients are responsible for purchasing licenses and game care.  If a client needs assistance with recovering an animal, please don't hesitate to ask us.
